New Patient Information

1. Please print and complete the New Patient Form.

2. Please print and complete the Appointment and Financial Policies Form.

3. Bring these completed forms and any insurance cards to your initial visit.

Your First Appointment

At your new patient appointment, we will be performing a variety of services in order to assist in providing you with the best care possible. These services include a one-on-one patient interview with the dentist, medical history review, oral cancer screening, complete set of dental x-rays, and a thorough examination. After the exam, the dentist will inform you of any existing problems, and provide you with appropriate treatment options, as well as answer all of your questions concerning your dental health. On average, this appointment takes approximately an hour and a half.
